The person who the principal is authorizing to make medical decisions in their behalf is called the surrogate. Surrogate is defined as &#8220;someone who takes the place of another&#8221;. A Designation of Health Care Surrogate form may also name a backup surrogate in case the first named surrogate is unable or unwilling to act in behalf of the principal. The surrogate cannot make medical decisions or consent to medical treatments on behalf of the principal until the principal is no longer able to make their own health care decisions and consent to medical treatments. The principal&#8217;s inability to provide informed consent is determined by the principal&#8217;s attending physician after assessing the principal&#8217;s condition. The surrogate&#8217;s authority ends when the principal regains the ability to make their own medical decisions and consent to medical treatments.</p>
<p style="text-align: justify;">When a surrogate is notified by the health care facility that they are now authorized to act on behalf of the principal due to the principal&#8217;s present condition and the surrogate is not the husband or wife of the principal, the surrogate must notify the principal&#8217;s spouse or adult children of the principal&#8217;s designation of health care surrogate. The surrogate may also begin to make health care decisions on behalf of the principal now. If the principal had previously informed the surrogate as to what types of treatments he or she would or would not consent to under particular circumstances, the surrogate should make their decisions based on the principal&#8217;s wishes. If the surrogate should have to make a decision about something that was not discussed with the principal, the surrogate should make their decision considering the principal&#8217;s best interest when deciding. Before the surrogate makes a decision that was not discussed with the principal beforehand, the surrogate may ask for medical information and advice from the principal&#8217;s health care providers and have access to the principal&#8217;s medical records. The surrogate not only has the authority to consent to medical treatments for the principal, the surrogate may also decline suggested medical treatments or ask that medical treatments being administered to the principal be stopped or removed as well. The surrogate may also request that the principal&#8217;s medical records be shared with another medical facility so that the surrogate could get a second opinion from a different physician and even request that the principal be transferred to a different health care facility.</p>

For example, registered nurse jobs are slated to grow much faster than average, with the creation of over half a million new nursing jobs in the next several years. This is in addition to the hundreds of thousands of jobs that will become available due to retirement in the existing population of working nurse personnel.</p>

It has led to such buzz terms as &#8220;Obamacare&#8221; and &#8220;death panels.&#8221; Beyond anecdotal horror stories (which can erupt from any type of healthcare system), it is important to note what the main problems with the current system are. This article will focus on the four areas that have generated the most heated debate. The reader can probably think of a few others as they are going through this, but I will venture a guess that most issues could fit under these umbrellas with minimal effort.</p>
<p style="text-align: justify;">Cost: One could make a strong argument that cost is complaint number one, two, three, and so on with the health care system. Costs associated with healthcare have risen at a rate far greater than inflation or wages. They argue that they want the freedom to choose their own physicians and treatments, and suggest that government cannot know what is best for them. These people argue that preserving the current system with improvements to provide better insurance coverage for citizens who remain uninsured or under insured for their medical care needs is the only reform that is needed.</p>
<p style="text-align: justify;">Those who believe health care is an individual right support a universal health care system with the argument that every citizen deserves to have access to the right care at the right time and that a government&#8217;s responsibility is to protect its citizens, sometimes even from themselves.</p>

<p style="text-align: justify;">Examination of some of the common ethical decision making theories can provide a foundation for a different perspective than one that is solely concerned with individual rights and freedoms.</p>
<p style="text-align: justify;">Ethical decision making requires that specific questions be answered in order to decide on whether intended actions are good or morally correct. Here are some questions that could be used in ethical decision making for health care reform.<span id="more-13"></span></p>
<p style="text-align: justify;">What action will bring the most good to the most people?<br />
What action in and of itself is a good act and helps us to fulfill our duties, obligations, and responsibilities to each other?<br />
What action in and of itself shows caring and concern for all citizens?</p>
<p style="text-align: justify;">As the answer to all these questions, universal health care can always be considered the right thing to do.</p>
<p style="text-align: justify;">The United States is in the most advantageous position there is when it comes to health care reform. They are the only developed country without a national health care system in place for all citizens. They have the opportunity to learn from the mistakes that have been made by all the other countries that have already gone down the universal health care road. They have an opportunity to design a system that can shine as a jewel in the crown of universal health care systems everywhere.</p>
<p style="text-align: justify;">However, all ethical decision making is structured around values. In order for universal health care to be embraced by all citizens in the United States, they will first have to agree to the collective value of equity and fairness and embrace the goal of meeting their collective responsibility to each other while maintaining individual rights and freedoms. That may prove to be the most difficult obstacle of all.</p>

Primary care doctors are usually the first point of entry when there are any health concerns before referrals to any other appropriate health establishment if necessary. There are thousands of insurance companies that cover private health insurance and up until fairly recently it has been very hard to get health insurance to cover autism because it is risky and treatment is very expensive. This has recently changed due to new legislation but availability can depend on whether a particular State has enacted autism insurance legislation or coverage for government funded health programs such as Medicaid.</p>
<p style="text-align: justify;">The causes of autism are not readily known or available. The U.S. Center for Disease Control and Prevention (CDC) report that autism and related disorders are more common than previously thought. There is an increase in those being diagnosed and 3-4 times as many boys than girls are affected. On average one in 110 children born in the U.S. have autism. One in 70 boys and one in 315 girls are affected. It is thought by professionals that the increase is due to a wider definition of the spectrum.</p>
<p style="text-align: justify;">It is essential to get an expert diagnosis in order to access the different services and treatments that may be available as it is beneficial to begin an early intervention program. An evaluation and assessment of the child may be done by a multidisciplinary team of professionals. Doctors who specializes in autism will observe the child, ask parents questions about the child&#8217;s development and behavior and do a variety of tests such as intelligence tests to evaluate the child&#8217;s strengths and weaknesses.</p>
<p style="text-align: justify;">The following are some examples of the types of people and places listed by the National Institute of Mental Health (NIMH) of whom to go to that will make a referral to, or provide diagnostic and treatment services (NIMH):</p>
<p style="text-align: justify;">Family doctors<br />
Mental health specialists such as psychiatrists, psychologists and counselors<br />
Community mental health centers<br />
Health maintenance organizations<br />

<p style="text-align: justify;">Once diagnosed, the quest to find affordable health insurance coverage to cover autism can begin. Around half of states currently have enacted autism insurance legislation which makes healthcare insurance coverage for more available, though it can be expensive. Recent laws have increased coverage for government funded health programs such as Medicaid for those on low incomes and disability is covered if it comes within the disability guidelines, so more families now qualify for assistance.<span id="more-11"></span></p>
<p style="text-align: justify;">There are all kinds of treatments and interventions available and a treatment plan can be devised and tailored towards the individual child. Different teams of specialists can evaluate such things as speech, communication and motor skills. The main ways of treating the child can be through:</p>
<p style="text-align: justify;">Behaviour Therapies and other types of therapies</p>
<p style="text-align: justify;">Applied Behavior Analysis (ABA) can be used to shape and modify behavior. Occupational Therapy is available to work on fine and gross motor skills, for example, and there are other therapies such as Speech Therapy.</p>
<p style="text-align: justify;">Individualized Education Plan (IEP) for school age children</p>
<p style="text-align: justify;">Parents are encouraged to be involved with teachers in setting targets or goals to be reached within the particular school year and describes any special support required in meeting them.</p>
<p style="text-align: justify;">Medication</p>
<p style="text-align: justify;">Currently, there are no medications available to treat autism, but there are supplements that can treat and manage some of the symptoms. Ritalin, for example, can be used to treat impulsivity and overactivity and there are other drugs that can be used to treat behaviors such as aggressive behaviors or repetitive behaviors.</p>
<p style="text-align: justify;">Though healthcare is fragmented in the United States, there is a wide range of therapies and interventions available for children with an Autism Spectrum Disorder. Accessing these can depend on the child having an expert diagnosis on autism and on what health insurance coverage the child has.</p>

<p style="text-align: justify;">Canada&#8217;s primary healthcare system provides services to individuals, families and communities. It also involves a proactive approach to preventing health problems and ensuring better management and follow-up once a health problem has occurred.</p>
<p style="text-align: justify;">These services are publicly funded from general tax revenues without direct charges to the patient.</p>
<p style="text-align: justify;">A patient may be referred for specialised care at a hospital or long-term care facility or in the community. The majority of Canadian hospitals are operated by community boards of trustees, voluntary organisations or municipalities.</p>
<p style="text-align: justify;">Healthcare services are mainly provided in long-term institutions, paid for by the provincial and territorial governments, while room and board are paid for by the individual; in some cases these payments are subsidised by the provincial and territorial governments.</p>
<p style="text-align: justify;">Healthcare services can also be provided in the home and/or community. Referrals to home care can be made by doctors, hospitals, community agencies, families and potential residents.</p>
<p style="text-align: justify;">These services, such as specialised nursing care, homemaker services and adult day care, are provided to people who are partially or totally incapacitated. <p style="text-align: justify;">Funding the system</p>
<p style="text-align: justify;">Like the NHS in Britain, Canada provides a good but not perfect system of healthcare. The level of coverage varies across the country and many Canadians have supplemental private insurance coverage through group plans, which covers the cost of these supplementary services.</p>
<p style="text-align: justify;">The Canadian Medical Association believes an estimated four million of Canada&#8217;s 33 million population don&#8217;t have a family doctor and more than one million are waiting for treatment.</p>
<p style="text-align: justify;">Canada has 2.1 physicians per 1,000 people, while Belgium has 3.9, according to the Organisation for Economic Cooperation and Development.</p>
<p style="text-align: justify;">Much of the resentment towards the healthcare system is caused by the fact that so much of Canada&#8217;s already high tax goes towards it. The average Canadian family pays about 48 per cent of its income in taxes each year and, while rates vary from province to province, Ontario, the most populous, spends around 40 per cent of its tax revenue on health, according to the Canadian Taxpayers Federation.</p>
<p style="text-align: justify;">The federation, which campaigns for tax reform and private enterprise in healthcare, believes the system is suffering serious financial challenges. It calculates that by 2035, Ontario will be spending 85 per cent of its budget on healthcare.</p>
<p style="text-align: justify;">The federal government and most provinces acknowledge there&#8217;s a crisis: a lack of physicians and nurses, state-of-the-art equipment and funding. In Ontario, more than 10,000 nurses and hospital workers are facing layoffs over the next two years unless the provincial government boosts funding, says the Ontario Hospital Association, which represents healthcare providers in the province.</p>
<p style="text-align: justify;">In 1984 Parliament passed the Canada Health Act, which affirmed the federal government&#8217;s commitment to provide mostly free healthcare to all, including the 200,000 immigrants arriving each year. The system is called Medicare (no relation to Medicare in the United States).</p>
<p style="text-align: justify;">Despite the financial burden, Canadians value their Medicare as a marker of egalitarianism and independent identity that sets their country apart from the United States, where some 45 million Americans lack health insurance.</p>
<p style="text-align: justify;">In 2000 The World Health Organisation ranked Canada 30th in the provision of public healthcare and the United States 37th. France&#8217;s system was ranked the best, followed by Italy, Spain, Oman and Australia.</p>
